I have tried this on two different clients (Vista 64 and XP 32), the result is the same. I have MT 2009 V. 7.12.
The error is:
The upgrade patch cannot be installed by the Windows Installer service because the program to be upgraded may be missing, or the upgrade patch may update a different version of the program. Verify that the program to be upgraded exists on your computer at that you have the correct upgrade patch.

I have seen this error on occassion. The workaround to this is to download/install the full version.

Is it possible to simply install over the existing installation or do I have to uninstall first?

It's not necessary to uninstall first as the installer will perform this for you.
